Form CDL-7, Texas Commercial Driver License Self-Certification Affidavit , is a form that needs to be filled by all Texas Commercial Driving License (CDL) holders to comply with regulations that took place on January 30, 2012, with full compliance due by January 30, 2014. The original deadline has been extended to January 30, 2015. Failure to maintain a current Medical Examiner's (ME) certificate with the Department of Transportation (DOT) after January 2015 will result in the loss of your CDL.

Texas Form CDL-7 Instructions

A CDL-7 Form must be provided to the Department of Public Safety (DPS) specifying the type of commercial vehicle you are planning to drive, stating that you will only operate commercially in 1 of 4 categories. This process is called self-certification.

After receiving your CDL-7 through the DPS, the State Driver Licensing Agencies will add your CDL-7 status and your ME information to your CDL record. Often your ME certificate is also referred to as a medical card. Generally, it has to be renewed every two years. None of the federal standards for the physical qualifications of SDL drivers have changed.

To complete the CDL-7 Form, determine what kind of commerce you will be operating in - interstate or intrastate. Are you excepted or non-excepted from any State and Federal requirements? Select one of the following categories that apply to you:

• Interstate non-excepted . For an Interstate non-excepted driver that needs to meet the Federal DOT ME requirements.
• Interstate excepted . For an Interstate excepted driver that does not need to meet the Federal DOT ME requirements.
• Intrastate non-excepted . For Intrastate non-excepted driver that needs to meet the State DOT ME requirements. Intrastate excepted:

Provide general information in the top section of the form. Include your CDL number and SSN. Select the correct category of commercial transportation. Expired Medical Variance (waiver for certain medical conditions) or DOT ME Certificates must be renewed and submitted to the DPS.
